Absent minded developers

There is a part of Australia called Tasmania which a separate island . Travel to the mainland is via ferries which carry vehicles, freight and people. The service is vital.

The Tasmanian government ordered two new you beaut ferries at $850 million each . They totally forgot to provide any site which could berth these large new ferries.

It makes me think about similar lack of forsight in software development.
The Linux kernel team has made considerable effort to provide driver modules for almost every hardware component, with the possible exception of some difficult graphics cards.
The BSD developers, on the other hand, have made less progress with driver modules.
There are consequences… almost all desktop Unix is Linux, not BSD.
That may be changing, Freebsd have announced they are going to make a special desktop effort. We wish them well. Better late than never, they say.

I am sure there are other such cases of sheer lack of thought re what is required to make the whole thing work.

Can you think of any?
